Abstract

#Gellan gum-based hydrogels have been proposed for biomedical applications including bone repair and cartilaginous tissue regeneration. This #polysaccharide has the ability to form stable gels without the use of harsh reagents and offers excellent #gel stability and flexibility. Gellan gum can function as a #minimally invasive injectable system, gelling inside the body in situ under physiological conditions and efficiently adapting to the defect site. This work presents an overview of the recent advances in the research on gellan gum for biomedical applications.

Abstract

The #anterolateral thigh (ALT) flap has gained popularity in #reconstructive surgery in recent years as its advantages have come to be widely recognized. We report the senior author’s experience over a five-year period with the use of the ALT flap. The ALT flap is a highly #versatile flap that can be used to reconstruct a variety of defects. The flap’s concealed donor site scar, decreased need for skin #grafting, decreased donor site morbidity and great versatility make it a first-line option for many different types of defects.

Abstract

#Sarcopenia is the progressive involution of #musculature, with a decrease of power and with limitation of physical and cognitive functions, including isolation, loss of independence and impairment of the quality of life. It is a gerontology syndrome, a component of age-related frailty. Because of the #dismobility there is a hazard of falls and fractures with a significant #mortality risk. This article reviews the relation of sarcopenia to aging, definition and epidemiology and #pathophysiology of sarcopenia with publications and recommendations for screening, diagnostic tools and #therapeutic modalities.

Abstract

#Syngnathia is a rare anomaly that involves fusion between the #maxilla and the #mandible. The fusion may be due to adhesions of the soft tissue between the bony fusion between maxilla and mandible. The latter is a very rare incongruity. These children present in the neonatal period with difficulties in the airway protection and feeding problems. Surgical management involves break down of the adhesions in the first few days of life. The prognosis is generally favorable. Congenital #craniofacial defect encompass 20% of congenital birth defects [1]. The defects of the fusion can present in a severity from single mucosal band (synechiae) to complete bony fusion (Syngnathia) [2]. Congenital maxilla-mandibular syngnathia, (fusion of the jaws), is a rare condition that has a broad spectrum of presentations [3].

Abstract

This report describes the results of clinical, cytological and #histopathological findings associated with #subcutaneous extraskeletal osteosarcoma of a 05 years old Australian Silky Terrier. The tumor was located on the head above the frontal and parietal part of the skull. A definitive diagnosis was based on histological examination of the mass. Even though #chemotherapy was suggested owner declined it therefore wide surgical resection of the tumor was performed with appropriate supportive therapy. However, the dog died few months after the initial presentation. To our knowledge, this case report is the first description of extraskeletal osteosarcoma of the head in a dog in Sri Lanka with the history of swelling on the head, lethargy, head pressing against objects and anorexia. Extraskeletal osteosarcoma should be considered in the differential diagnosis of soft tissue swelling in head in dogs. It is highly recommended to use advance #diagnostic aids in order to rule out primary #bone tumors.

Abstract

A system and method for the non-invasive portable #ultrasound-based measurement system of the Cardiovascular blood pressure is presented, wherein the blood pressure measurement model is calibrated to the simultaneous direct measurements by pressure transducers during the #catheterization. The model is based on the image time series processing estimating the changes of the oscillating traceable regions via the introduction of the new #ultrasound generalized M-Mode and the notion of the derived image. Once calibrated the system is ready to recalculate the pressure in the calibrated region, such as Left Atrium according to the subsequent non-invasive ultrasound measurements. The system has been successfully tested on animals (sheep) under subsequent #catheterisations.

Abstract

As a major #excitatory transmitter, glutamate plays an essential role in the functioning of the central nervous system [1]. Glutamate has an immediate impact as an #excitatory amino acid and has a role in long-term #neuronal potentiation, as a proposed molecular substrate for learning and memory. This neurotransmitter is also known to participate in many metabolic pathways and it plays a pivotal role in the #pathophysiology of several neurological diseases-including epilepsy, acute central nervous system trauma, and #hepatic encephalopathy [1]. Not only does glutamate play a role in the functioning of the central nervous system, but it is also believed to have an influence on knowledge acquisition and development in humans [2].

Abstract

Like many types of medicine, #local anaesthesia has very interesting origins. It seems slightly mad today that substances were more likely to associate with youth rave culture than our local GP, were once, not only legal, but readily available as analgesics for a wide range of ailments. At the #Anaesthesia Heritage Centre, we have cocaine and morphine ‘#tabloids’ for example, which could have been used for various pains; cocaine is even known to have been prescribed for morning sickness. Cocaine was the first substance noted for its local anaesthetic qualities. Coca leaves were first brought to Europe from South America around the mid-1800s and cocaine was isolated soon after by German chemist Albert Neimann. It was psychoanalyst, Sigmund Freud, who suggested cocaine to his friend and #ophthalmologist, Carl Koller.

Nano Medicine - BJSTR Journal

Abstract

The Merriam-Webster’s Collegiate Dictionary defines museum as “an institution devoted to the procurement, care, study, and display of objects of lasting interest or value” as well as “a place where objects are exhibited.” To this end, the present paper deals with that of the Royal College of Surgeons of England, although it seems to be obtainable by even an individual. The Merriam-Webster’s Collegiate Dictionary eminently defines the museum as “an institution devoted to the procurement, care, study, and display of objects of lasting interest or value” as well as “a place where objects are exhibited” [1]. It seems that an individual may even own it, seeing that Edward Seymour [2] revealed that “In the collection of preparations of morbid parts in the possession of Mr. Brodie, there is a specimen of the #fungus hæmatodes of the liver (#tubera diffuse), #scirrhus of the breast, and a disease of the uterus …” In this context, it is of interest that England was mentioned when tracing the source of inspiration for the development of collections which grew as Harvard’s Famous Warren Anatomical Museum [3].

Abstract

There is always more than meets the eye in a #fractal frame of reference. Almost three quarters of our globe is home to another world we are only beginning to understand. Yet what has been very obvious from our earliest investigation of the sea is that it reveals a complex reality alien to us. The sea is a world in and of itself. Though it provides the very fabric of existence of the life it sustains, the combined network of oceans that blankets our planet goes unnoticed by its #inhabitants due to its scale and the fact that these waters are the very space (i.e., fluid) these life-forms live in and navigate through moment by moment. As it is with the sea life, so it is in our universe. We, like our #marine counterparts, fail to notice the sea of our own reality.

Abstract

Objective: #Hypertension is a modifiable #multifactorial risk factor that has been associated with cognitive impairment. The current study investigated the relationship between blood pressure levels and executive function in older Mexican Americans.
Methods: Data from 349 cognitively normal participants from the #Health and Aging Brain Study among Latino Elders were analyzed to compare executive function status in those with and without a diagnosis of hypertension. Diagnosis of hypertension were based in self-report, medication status, and examination results. Executive function was assessed with Trails B education adjusted scale scores. Diabetes, #dyslipidemia, and #abdominal obesity were also entered in the models with age as a covariate.

Abstract

#Lower back pain can be a debilitating condition that is often caused by ligament and #fascial sprains, and #muscle strains. Even though the diagnosis may be a herniated disc, facet #arthropathy, degenerative disc disease, spinal stenosis, scoliosis, #spondylosis, spondylolisthesis, or other pathology, we have found for the past twenty years, with thousands of successfully treated patients, that the actual pain generator may not even be noted in the diagnosis. With the risk of adverse complications of surgery and ineffectiveness of epidural injections, bone marrow concentrate (BMC) offers a promising treatment to treat lower back pain. BMC contains #mesenchymal stem cells that have the ability to differentiate into muscle, cartilage, and bone, in addition to releasing trophic factors that enhance #tissue regeneration.

Abstract

#Occupational splashes with blood and body fluids, cuts with contaminated sharp objects and needle prick injuries (SSNIs) are the most prevalent accidents in #healthcare settings [1-3]. Such accidents and injuries have the potential of transmission of #blood borne pathogens to healthcare workers (HCWs). The US Center for Disease Control estimated that the occupational risk of transmission of #Hepatitis B Virus (HBV) amongst HCWs was 22-31%, Hepatitis C virus was 0-7% and Human Immuno Deficiency Virus (HIV) 0.09-0.3%. But, the risk of transmission of HIV alone was higher (0.3%) in #percutaneous injuries compared to 0.1% for splashes to the mucous membrane and < 0.1% in non-intact skin [4].

Abstract

The relationship between circadian rhythm and autonomous cortisol secretion of #adrenocortical lesions associated with #Cushing’s syndrome is still unknown. We show here that in Y1 adrenocortical tumor cells, circadian rhythm of Per2 mRNA expression was in a 24 h manner, while that of #steroidogenic-related genes such as P450scc mRNA and Ad4BP/SF-1 mRNA was asynchronous with Per 2 mRNA, compared to normal mouse adrenal primary culture tissue which show #synchronized expression of the three genes. #Deletion mutants of Ad4BP/SF-1 promoter activity in Y1 cells show that the E-box element functioned in a circadian-like pattern which was abrogated in promoter constructs which lack the E-box. Rhythm of upstream factor-1 (USF-1) mRNA correlated with Ad4BP/SF-1 mRNA expression, which is known to bind the E-box of Ad4BP/SF-1 promoter.

Abstract

#Bladder cancer is one of the most common #malignant tumors in the human body. At this stage, there is no significant progress in treatment. Therefore, early diagnosis of bladder cancer is particularly important. At present, the urine #cytology screening, #cystoscopy and pathological analysis are the most effective method for bladder cancer diagnosis. Due to the low specificity of urine exfoliated cells and cystoscopy is an invasive examination which is difficult for many patients to accept, there is an urgent need to find biomarkers of bladder cancer and to elucidate the mechanism of progression of bladder cancer for later treatment. Recently, some scholars have focused their attention on #exosomes, which are a kind of nano-sized vesicle structure secreted by cells. They are distributed in various body fluids such as blood, saliva and urine. As an important class of #intercellular communication molecules, exosomes contain a variety of biologically active components, which can play a regulatory role in the human body in a variety of ways. This paper is mainly about the introduction of bladder cancer exosomes.

Abstract

#Jejunostomy is the recommended route for enteral nutrition in certain patients. Various methods and equipment had been proposed to complete the operation #laparoscopically. The method we proposed is based on the triangular rule of trocar orientation with typically available tube and equipment. It minimized the wounds to one 11mm trocar for the laparoscope plus two 5mm trocars for #intracorporeal suturing, by creating the jejunostoma with straightforward surgical blade penetration. Sixty-nine patients underwent the operation as either a solitary procedure or a part of the operation based on preoperative indications. The majority of the patients received feeding within 3 day after operation (84%) without major complications. Fifteen patients experienced #tube dislodgement or occlusion during the follow up but did not encounter difficulty while exchanging new tubes. The method we proposed could be done with readily available equipment and minimized wounds and did not yield major complications.

Abstract

Summarizing we can describe the following logical chain. Spontaneous appearance of “living” states in #physico-chemical systems is impossible in sense that it is extremely improbable. However, with the help of the biological systems this process constantly occurs. Hence, the consciousness make re-normalization of probabilities of states and it is topologically determined by higher structural level (#omnicausal determinance). Such omnicausal re-normalization of probabilities, as was mentioned although not evidently by many authors, is a characteristic for all levels of the biological hierarchy from a #neuron up to the biosphere.

Abstract

Surgical closure of cardiac defects via a full mid-line #sternotomy has been considered the gold standard for over 50 years. Due to the boom in interventional cardiology and the proliferation of techniques such as laparoscopy or #thoracoscopy in other surgical specialties, several teams have adopted alternative approaches to median sternotomy [1-7]. The initial enthusiasm, fuelled by the technical advances of the past twenty-five years, stood against the reticence of those with a traditional approach. Doubts regarding the complexity and operative time of the technique, as well as its outcomes, prevented minimally invasive surgery from becoming widely used in congenital heart disease. Among the most frequent alternative approaches (Figure 1) we find: lower #mini-sternotomy [8-11], right sub-mammary [1,12-16], #postero-lateral thoracotomy [17,18] and #right axillary incisions [19-23]. Main advantages are #cosmesis and earlier recovery, as well as saving blood products and lower infection rates.

Abstract

Unintended ingestion of foreign body into #gastrointestinal (GI) tract is not an uncommon event, and it complicates with perforation, obstruction or #intra-abdominal infections in certain scenarios. The incidence of GI tract perforation developed after foreign body ingestion is less than 1% [1]. Hepatic #pyogenic abscess resulting from foreign body migration is even rare [2]. Among the reported patients, most of them failed to recall the time of ingestion and the type of foreign body. The disease severity of hepatic abscess ranged from indolent chronic infection to #life-threatening septic shock, but the treatment was similarly composed of adequate drainage and targeted antibiotics [3]. Regard the rare but specific spectrum of secondary hepatic abscess induced by #foreign body ingestion, removal of the migrated foreign body is crucial in controlling the infection [4].
