Abstract


Results of recent studies showed that the #insulin growth factor (IGF) pathway might be implicated in the development of epithelial #ovariancancer (EOC). Moreover, it was demonstrated that EOC cells with high #microsatellite instability (MSI-H; non19/non19) accumulate #mutations in the IGF-1 #gene. The aim of our study was to analyses the (CA)n repeats polymorphism in the P1 promoter region of the IGF-1 gene in EOC and healthy volunteers.For identification of (CA)n repeats PCR and DNA sequencing were utilised. Serum blood concentrations of IGF-1, IGFBP-1, IGFBP-3 were determined with ELISA. IGF-1 expression was assessed by #immunohistochemicalanalysis. According to the obtained results biological samples were classified as microsatellite instability-high (MSI-H), MSI low (MSI-L) and microsatellite stable (MSS).

This paper reviews and discusses the recent studies of natural #floodmanagement (NFM) and its associated microbial risks in the UK and suggests set of numerical modelling approaches for their respective investigation. This study details the importance of precise #numericalrepresentation of the NFM to flood inundations and #microbialrisks caused by NFM measures. Possible future numerical advancements of the numerical modelling for the NFM and #microbialactivities are also discussed here.

Family #Medicine is a wonderful profession, where the beauty of all medical specialties can be faced, if using your head, heart, intuition and hands properly. This work-field allows us to be the #witnesses of a real diagnostic pearls and requires doctors to become investigators. Research work in rural, solo, primary #healthcare practices is not an easy work to do - isolation, burden of the poor and diseased, overload, lack of resources and being under-valued by the National Government are the key factors of the delayed development of research in this field. Contemporary views show, that research develops by itself and we cannot stay indifferent to it. The up-to-date study methods are limited, which make doctors to look for the ways to grow up with and co-develop. For instance, a primary #hypothyroidism is not an everyday #diagnosis to meet in a family doctor’s practice. It is actually quite a rare pathology.

Abstract

SYNTHESIS AND BIOLOGICAL EVALUATION OF 1-(5-((9H-carbazol-9-yl)methyl)-2-methyl-1,3,4-oxadiazol-3(2H)-yl)ethanonering were synthesized by the #condensation of 2-(9H-carbazol-9-yl)acetohydrazide with 2-(9H-carbazol-9-yl)-N’-ethylideneacetohydrazide and acetic anhydride. To this reaction was subjected. It forms 2-(1-((4-acetyl-5-methyl-5-(trifluoromthyl)-4,5-dihyro-1,3,4-oxadiazol-2-yl)methyl)-1Hindol -3yl)-3-(p-tolyl)thiazolidin-4-one.The structure of these newly #synthesizedcompounds were characterised by 1H NMR,13CNMR ,Mass ,IR, and #elementalanalysis.

Abstract

Here we describe a 67 year old female affected by hairy cell leukemia (HCL) treated with 2-CDA, who developed prolonged bone marrow failure. Severe and persistent #pancytopenia led to lethal pulmonary #aspergillosis on day +85 of treatment. We have then reviewed literature in order to better understand occurrence of #neutropenia and infections in this kind of patients. The most prominent side effect of 2CDA has been reported to be #myelosuppression, but only one irreversible failure has been described until now.imately dead from serious complications. The rare and special #gastrointestinal lesions after acute diquat poisoning is worth for us to explore.

Abstract

A 24-years-old male with massive diquat #ingestion complicated by the development of #gastrointestinalulcer perforation, intestinal#hyperpiesia, liver failure, renal failure is described. The patient ultimately dead from serious complications. The rare and special #gastrointestinallesions after acute diquat poisoning is worth for us to explore. Diquat (1,1’-ethylene-2,2’-bipyridilium) is a non-selective #bipyridylherbicide similar to paraquat [1]. Since the paraquat solution was banned in China since September 7, 2016, the diquat has gradually become a major substitute for the herbicide market [2].

Abstract

Objective: We thought the stable #implant construction may affect outcome for elderly #hipfracture so assessed the #patients for one year as a consecutive prospective study.
Method: We include 83 patientsall above 60 years old patients who came to our center with unstable #intertrochantericfracture in one-year admission.at the end we assess the mortality rate and compared with other study.
Conclusion: It seems stable implant has a little role in mortality rate,but in one year follow up we had 24% #mortality and 50% readmission because #comorbidity exacerbated which compare to other studies had no deference(p<0.01)

Abstract

#Peripheralnerve injury can be surgically treated, but functional recovery is still unsatisfactory. That is why additional methods that can be applied after surgical repair and which modulate nerve #regeneration are sought, this regeneration involving both the morphological recovery of the #axon and the myelin sheath, but also the functional, physiological recovery of the peripheral nerve. Thus, in recent years, experimental studies have been undertaken #in vivo and #in vitro, but also in clinical trials involving #electricstimulation, low-level laser therapy and #pharmacotherapy. But many of these attempts are not yet standardized and well established, although their effect on the #anatomicalfunctional recovery of the peripheral nerve is indisputable. In this review we present a brief overview of the main results obtained in these researches in recent years.

Abstract

Stimuli-responsive #nanogels have achieved great progress in#biomedicalfields for #clinicaldiagnosis and #therapy due to their porous structures, large surface area, good biocompatibility [1]. Recently, much attention has been focused on the synthesis of stimuli-responsive nanogels with optical maker in polymer network, therefore, realizing the combination of #biosensing, #bioimaging and controlled drug delivery [2]. Such hybrid nanogels have great potential for disease diagnosis and imaging-guided cancer therapy since the #fluorescent imaging not only precisely monitor the change of biosignal in vitro and in vivo, but also identify the location of the tumor region and monitor the #treatment progress [3].

Abstract

Several studies suggested that platelets might play a crucial role in #pathogenesis of inflammatory #boweldisease (IBD) [1,2]. There is solid evidence demonstrating that platelets, in addition to their traditional role in #hemostasis, can also function as potent #proinflammatorycells [3]. Upon activation, platelets secrete a large number of biologically active molecules able to activate an inflammatory process. At the same time, the morphology of platelets changes and their mean volume (MPV) reduces [4]. Low MPV can be a marker of activity of IBD [5]. However, the way the functional properties of  #platelets change, primarily their #aggregation capacity, remains unclear. Small platelets have lower functional abilities and larger platelets are metabolically and #enzymatically more active [6,7].

Abstract

Most of #pediatricpatients with type 1 #diabetes are currently treated with intensive #insulintherapy including multiple daily injections of insulin and insulin pump. Various reports have shown insulin pump is effective to improve #glycemic control with decreasing #hypoglycemia. However, insulin pump may be hazardous when pump problems occur, and the correspondence is wrong of problems. Even short interruption of insulin delivery can cause rapid rise in blood ketones accompanied by elevation of #bloodglucose. It is noteworthy that insulin pump failure can be an important risk factor for DKA during the management of diabetes in pediatric patients with type 1 diabetes.Diabetic #ketoacidosis (DKA) is known to be a lethal condition, and the mortality rate of DKA in children and adolescents is reported to be 0.15-0.30% [1].

Open Access Journals on Surgery

Abstract


The #biomodulation and the #biostimulation of Low Level Laser Therapy (LLLT) promote wound healing, dentine repairing processes and stimulate the biological activity of human Periodontal Ligament Stem Cells (HPDLSCs), human Dental Pulp Stem Cells (HDPSCs), human Bone Marrow Stem Cells (HBMSCs) and human Adipose Stem Cells (HADSCs). The aim of the study was to review the literature on the LLLT effects on DPDLSCs, DDPSCs, DBMSCs and HADSCs. The electronic search was performed collecting articles in the PubMed, Medline, #Scopus, Lilacs and Google Scholar databases from January 1990 to January 2018. LLLT was able to significantly increase HPDLSCs, HDPSCs, #HBMSCs and HADSCs proliferation rate, stimulating 3osteogenic differentiation and opening new possibilities in the tissue regeneration. However, the research background on this topic showed high heterogeneity and the in vitro studies cannot always mimic the in vivo conditions. Thus, further clinical studies should be performed to determine the appropriate dose of irradiation in different patients.

Abstract


Objective: Analyze the interaction between dysphagia and the #nutritional implication on Neuron Motor Disease patients.
Method: Through observational, #transversal, prospective and analytical research, 59 patients, 42 (71,18%) were patients with #Amyotrophic Lateral Sclerosis (47,62% males and 52,38% females) and 17 (28,82%) were patients with Progressive Bulbar Paralysis (7 (41,17%) males and 17 (58,83%) females). Patients underwent dysphagia and nutritional assessment and other scales like: Functional Oral Intake Scale, Functional Rating Scale (ALSFRS-R and EGELA) and #respiratory assessment (cough peak flow).
Results: 100% of the patients with #bulbar involvement showed changes in oral and pharyngeal phases, while ALS patients showed changes in oral phase (66,67%) and pharyngeal phase (73,80%). The nutritional assessment showed malnutrition in 35,71% of the patients with ALS and 23,52% of the patients with PBP. Patients who used other alternative type of feeding: 28% (ALS) and 41,17% (PBP).

Abstract


#Cardiorenal syndrome (CRS) can be defined as a #clinical-pathological disorder in which a primary insult in the kidney or heart initiates a series of functional and #morphological secondary dysfunctions with responses in the other organs. A 63-year-old male patient with heart failure secondary to ischemic heart disease presented decompensation associated with #hyperkalemia, evolving cardiogenic shock and acute kidney injury (AKI). In this case, an early use of inotropic medications to correct the low-output syndrome, associated with the identification of the AKI, was the differential that contributed to the positive outcome of the case.

Abstract

Introduction: #Chestpain (CP) is a frequent symptom in emergencies worldwide, presenting as a diagnostic challenge for the emergency room, due to the multiplicity of etiologies, and the #diagnosticfluctuation between low and #high-riskdiseases. The optimized door-to-ECG time is a target and excellent index to be reached in the approach of the patient with TD. Rapid inflow to the hospital until the first EGG can be performed can accelerate the #diagnosis and #therapy in the patient with DT by determining a risk minimization.
Objective: To evaluate the impact of insertion of a rapid inflow of patients with chest pain in emergency on optimization ECG time.
Method: Door-to-#ECG time was evaluated before and after the insertion of a rapid inflow of the patient with CP to the hospital until the first ECG was performed.

Abstract

The aim of the present study is to draw attention to the rare association of maple syrup urine disease and alternating #hemiplegia.We present a case of a 3-year-old boy adopted 3 months before the onset of alternating hemiplegia events, with development delay, had investigation with MRI and Urinary #organic acid suggestive of MSUD. Started the treatment restriction of leucine, #isoleucine and #valine intake and #thiaminesupplementation, with resolution of his #neurology symptoms.

Abstract

Objectives: Discuss the #hypothesis of #HIVinfection to reduce the risk of #MultipleSclerosis.
Method: Non-systematic review using a Google Scholar platform of articles with the keywords: “HIV” or “#Human Immunodeficiency Virus” associated with the term “multiple sclerosis” in the title of the journal.
Results and Discussion: 44 articles were used in English language for this manuscript.
Conclusion: There are few cases of patients with HIV and multiple sclerosis. The HIV infection may decrease risks of multiple sclerosis. The study of this association may contribute to the understanding of the #physiopathology in #inflammatorydiseases of the #central nervoussystem and the use of drugs for HIV infection in multiple sclerosis.

Abstract

Objective: make a review about the risk of #seizures related to the use of #antibiotics
Method: We perform a non-systematic review using Google Scholar platform approaching the relationship between seizures and the most used antibiotics classes in #clinical practice. Results and Discussion: 34 papers in English language were used for this review.
Conclusion: Rarely antibiotics may cause seizures, mainly in patients with renal #dysfunction, hepatic or previous #braindisease. It is important to learn about drugs at greatest risk of seizures in each class of antibiotics. The treatment involves the use of correct #medications, primarilygabaergic #drugs, avoiding ineffective #anticonvulsants.

Abstract

Background: Turnover time is still not negligible in ensuring efficient operating room(OR) management. A prospective study was performed to evaluate the effect of #instructionalsupervision by an OR assistant on reducing turnover time.
Material and Methods: We prospectively included surgical cases undergoing turnover operation after the first case from two ORs in a single center between Mar 2015 and Feb 2016. Cases in one OR received the instructional supervision by an OR assistant. Cases in another OR received not. The primary endpoint was the percentage of #surgicalcase with turnover time ≤ 45min. Other #endpoints were turnover time, the percentage of the teamwork score of OR staffs ≥4 point and the percentage of #patient satisfaction score≥4 point.
