Abstract

Background: #Trypanosoma lewisi is an #extracellular hemoflagellate protozon of rats. T lewisi was not considered a pathogen for human, However, cases of human trypanosomiasis caused by T. lewisi have been reported
Objective: There is little data on the prevalence of T. lewisi in #Rattus norvegicus rats in Egypt, so this study aimed to determine the prevalence of T. Lewisi in Rattus norvegicus in Abu Rawash, Giza, Egypt from November 2016 to October 2017.
Materials and Methods: 117 rats were trapped around human population in Abo Rawash, Giza, Egypt. Thin and thick blood films were prepared and examined microscopically. Experimental infections of 8 white rats were done for confirmation of infection and assessment of parasitaemia.
Results: 19 (15.8%) out of 117 Rattus norvegicus were found to be infected with T lewisi. Experimental infection of 8 white rats confirmed the infection with Trypanosoma lewisi and showed boost in #parasitaemia till the 9th day then the level of #trypanosomes in blood was constant.
Conclusion: Trypanosoma lewisi is prevalent in examined Rattus norvegicus which reverse that the theory of high immunity of Rattus norvegicus will prevent infection with Trypanosma lewisi.

Abstract

#Primary leiomyosarcoma (LMS) of the bone is a very rare tumor with #smooth muscle differentiation on #immunohistochemical studies. Radiologically, LMS appears as an osteolytic lesion with aggressive features but no #pathognomonic radiological findings that helps making the diagnosis. We report one case of primary LMS of the femur with fluid-fluid levels (FFLs) on the MRI, the first case to our knowledge. We consider we should include primary LMS of the bone in the differential diagnosis of "FFL” lesions. LMS is a malignant tumor comprising of spindle-shaped cells with distinctive smooth muscle cell features [1]. They are approximately 7% to 10% of all soft tissue sarcoma [2]. Most of these tumors occur frequently in uterus and soft-tissues and more rarely in bone [3-5]. The first description of a primary bone LMS in a long bone was given in 1965 [1]. Since then, to the best of our knowledge, around 130 cases of primary LMS of bone have been reported, mostly small case reports that have focused on its #histological diagnosis [4]. The clinical behavior of osseous LMS is generally aggressive. Most of the published studies of LMS of the bone have reported poor prognosis with a 35% overall survival rate [6]. LMS diagnosis is established by the presence of #morphologically typical spindle cells with smooth muscle differentiation and the positivity of a smooth muscle actin and other muscle markers on tumor cells, such as #desmin and h-caldesmon [7]. Furthermore, the specific characteristic of primary bone LMS is a solitary osteolytic lesion with indistinct margins and cortical destruction with the absence of either osteoid or chondroid matrix [8,9]. Since its recognition as a distinct entity, to our knowledge no sizeable series has examined its imaging features. Furthermore, only a few studies about primary LMS of the bone with emphasis on MRI findings have been published [10]. We present the clinical features, radiological findings and pathological description of a primary LMS originating from the distal femur in a middle-aged lady. Best to our knowledge, this is the first case report of "FFLs” on MRI of a primary LMS of the bone.

Abstract

Background: #Stimulus-evoked EMG improves the safety during pedicle screw placement in the #lumbosacral spine. The reported reliability of stimulus-evoked EMG in the detection of pedicle breaches in minimally invasive lumbosacral fixation has been variable. Besides the influence from different #physiopathological conditions, current shunting may also contribute in this variable reliability due to the small incision during minimally invasive surgery. Using a traditional suction system, continues suction is not viable during tapping and pedicle screw placement in minimally invasive surgery.
Objective: The author proposes a new design of non-metallic dilators adapted to continuous suction during tapping and #pedicle screw placement in minimally invasive surgery with the aim of minimizing the possible current shunting and optimizing the stimulation accuracy.
Materials and Methods: The proposed nonmetallic dilators have 2 canals. The larger canal would be used for the passage of the tap and the pedicle screw attached to the screwdriver. The lumen of the smaller canal would be connected to the connecting tubing attached to a reservoir with #vacuum pressure. After a small skin incision was made, each dilator was placed into a piece of porcine meat. The main canal of both dilators was partially sealed to simulate the passage of the instrumentation devices. Porcine blood was continuously injected into the created cavity to simulate the concurrent bleeding.

Abstract

Technology from the Greek term #technologies which derives from the words "techno" meaning "art" and "logos" meaning "oration, spiel" is nowadays defined as "the result of theoretical knowledge in order to create objects-machines that could affect the ability of a species to control its environment and adapt to it [1]. Imaging is defined as "the visual representation of an image which is perceived by human senses or of a form-figure which could not be understood due to its spatial position" [2]. Anatomy is defined as "the medical branch that deals with the study, recording, and description of the structure of living organisms and their individual systems" [3]. The term "history" derives from the Greek word historian (Greek: "lOTOpta"), meaning "inquiry, knowledge acquired by investigation"). History is usually defined as the study of past events, particularly in human affairs [4]. As technology succeeded to #manufacture machines able to visualize in depth depictions of the human body, in the narrow area where those for different entities are meet, medical imaging, history and anatomy are being connected through technology to create both clinical and learning curves in medicine (Figure 1). Worldwide, human gross anatomy courses form the basis for the medical curricula, aiming to introduce the very core of anatomical concepts within the clinical context both for future and specialized #physicians. A solid understanding of the human's body construction is essential for all medical specialties [5]. Although expert medical professionals consider #anatomy to be the most relevant basic discipline during their daily clinical activity, students usually perceive anatomy as the more challenging subject, resulting to anxiety and learning difficulties [6-7].

Abstract

Systemic lupus erythematosus (SLE) is a chronic inflammatory disorder that can affect any organ tissue during disease process. SLE may influence the nervous system at multiple levels for which it is termed #neuropsychiatric systemic lupus #erythematosus (NPSLE). SLE associated myelitis is a sever but relative rare complication with a prevalence of around 1% among SLE population [1]. The neurological deficits of SLE associated myelitis comprise motor, sensory and autonomic dysfunctions. Motor symptoms include an acute or subacute #paraparesis that can involve the upper extremities, with initial flaccidity followed by #spasticity. Most patients have a sensory level. Typical sensory dysfunctions include pain, #dysesthesia, paresthesia or sensory loss. Autonomic symptoms comprise bladder and bowel incontinence, difficulty voiding, constipation or incomplete evacuation, and sexual dysfunction. The signs and symptoms depend on the level and the extent of spinal injury.

Abstract

In recent years, with the continuous development of science and technology in China, some #engineering technology has been widely applied to the medical field, and a lot of medical equipment has been derived, which has made a great #contribution to the further development of China’s biomedical industry [1].With the development of biomedical engineering in China, medical equipment’s has become more simplified, mechanized and #precise, and have made considerable progress in the following fields:
a) Robots: Medical robots have higher accuracy in diagnosing and treating patients. The usage of medical robots through surgeons’ manipulation can minimize the #surgical trauma suffered by patients.
b) Genetic Therapy: For example, the cell separator for purifying malignant marrow cells can achieve good results for some genetic diseases.
c) Family Self-Treatment: With the continuous improvement of living standards of Chinese people in recent years, they put forward higher requirements for simplicity of treatment. The family self-healing equipment such as #rehabilitation appliances can improve the convenience of patients’ rehabilitation by allowing patients to complete the rehabilitation work at home.

Abstract


In this study, coupled oxide 3ZnO:1TiO2 (3ZT) synthesized by sol-gel method with different weight percentage were incorporated in linear low- density #polyethylene (LLDPE] for antimicrobial application. The nanoparticles were #homogeneously distributed across the surface due to the casting method adopted in this work. The formation of heterojunction of 3ZT and crystalline nature of the phases and high quantity of surface oOH resulted in LLDPE #nanocomposites with higher adsorption water molecules property, thus causing a substantial improvement in #photocatalytic reaction. The antimicrobial results indicate LLDPE incorporated with coupled oxide 3ZT was active in inactivating Gram-positive; #Staphylococcus aureus (S. aureus] and Gram-negative; #Escherichia coli (E. coli]. S. #aureus was found to be more susceptible in killing as compared to E. coli, under visible light. The best inactivation up to 100% was able to achieve for both bacteria in LLDPE with 10wt.% under visible light due to more Zn ion and oOH release at a specific #incubation time.Microbial contamination poses a major threat to human health. Many diseases spread due to the bacterial infections, which cause significant economic and personal losses [1]. Hence, antimicrobial modification of surfaces to prevent growth of detrimental microorganism is highly desired. In biomedical devices such as #catheters, prosthetics and implants, surface microbial invasion can result in serious infection and device failure [2].

Abstract

The problem of the location of #glioblastoma (GB) stem cells (GSCs) and of their relationship with the microenvironment is discussed. Also in our experience, they are located in #perivascular and #perinecrotic niches. In the latter, as an alternative hypothesis to their origin by hypoxia through #Hypoxia Inducible Factor 1, they could represent the remnants of stem cells/progenitors that populate the highly #proliferative areas of GB where necrosis develops in avascular zones due to the imbalance between the high proliferation rate of tumor cells and the low one of endothelial cells. On the whole, tumor stem cells and tumor non-stem cells are in equilibrium between differentiation and stemness, regulated by the #microenvironment with the possibility of an interconversion of the cell statuses.One of the most credited hypotheses on #tumorigenesis is the origin of tumors from cancer stem cells (CSCs). These would be at the top of a cell hierarchy and responsible for growth, recurrence and resistance to therapies. There would be an equilibrium between CSCs and cancer non-stem cells through an interconversion from one to another [1].

Abstract

Tani et al. [1] earlier described fenestrae in #Golgi apparatus and endoplasmic #reticulum cisternae of human brain tumors. Tougard et al [2] found short discontinuous Golgi cisternae in tumoral prolactin cells in culture. #Petito and #Pulsinelli [3] described transformation of Golgi apparatus into large clusters of small vesicles without cisterns in cortical and striatal neurons following cerebral ischemia. Petito [4] showed transient swelling of Golgi apparatus in #perineuronal oligodendroglial cells in areas of ischemic neuronal necrosis. Blanchette-Mackie [5] found abnormal accumulation of cholesterol at the level of Golgi complex in fibroblasts of patients with Type-C Niemann-Pick disease. Palacios [6] observed beta- amylod precursor protein localization in the Golgi apparatus of normal and axotomized rat neurons. Lucassen et al. [7] reported increasing size of Golgi apparatus in human #supraventricular nucleus neurons in aging and in Alzheimer's disease. Pathological disorders of Golgi apparatus have been also described by Ghadially [8] in a variety of non-nervous diseases involving altered secretory activity (#albuminemia, lipoproteinemias), cell surface pathology (cancer) and viral diseases.

Abstract

PCR or #Polymerase Chain Reaction is a technique widely used in molecular biology laboratories. Since it was developed in 1983, many procedures use this technique as a basic principle, for example: forensic tests, diagnosis of diseases, #phylogenetic studies, identification of mutations, nucleotide sequencing, among others [1-3]. In our procedures of IGF-I diagnostic, we use the techniques of northern blot, western blot, Elisa, #immunocytochemistry and cytofluorometry, and the technique of PCR seems still the most rapid and useful in clinical laboratory [4,5] The IGF-I test is among the most important indications for precancerous diagnostic [6]. Therefore, the accessibility of PCR technique giving increased efficiency and rapidity has become a goal to establish a new methodology. The traditional thermal cycler consists of a metal plate or block that has the #Eppendorf tubes located at the top, and at the bottom is an electrical device that uses #Peltier technology to increase or decrease the temperature according to the requirements of the cycle. There are temperature sensors distributed below the block (Figure 1).

A 30-year-old #Ethiopian female presented to the emergency department with intermittent worsening left lower quadrant pain and bilateral upper extremity pain. Past medical history is significant for sickle cell #anemia. Physical exam including vital signs were unremarkable except for tenderness to palpation in bilateral upper extremity and left lower quadrant (posterior greater than anterior). Complete blood count revealed only mild anemia (Hemoglobin 10.1 mg/dL), but was otherwise unremarkable, as was the comprehensive metabolic panel. #Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were elevated at 71 mm/hr and 12.3mg/L.A 30-year-old Ethiopian female presented to the emergency department with intermittent worsening left lower quadrant pain and bilateral upper #extremity pain. Past medical history is significant for sickle cell anemia. Patient had normal LDH, no leukopenia/lymphopenia and no clinical signs.

Abstract

#Acinetobacter baumannii is a non-spore, non-motile, non-fer- mentable and aerobic gram-negative #coccobacilli which belongs to the #Moraxellacea family [1]. The most of this genus are opportunistic pathogens in both community and hospital acquired infections in #immunocompromised patients, especially in burn and ICU parts after Pseudomonas aeruginosa. These microorganisms have a significant role in various infections including bloodstream, pneumonia associated with ventilation, endocarditis, meningitis, skin, soft tissues, urinary tract and medical implant devices infection. The #Acinetobacter species are isolated from numerous sources, such as soil, water, animals and human tissues. Mentioned infections have been effectively treated with traditional antibiotics in the last three decades [2-4]. However, recently the resistance to the most classes of antibiotics such as broad-spectrum penicillin's, cephalosporins, #carbapenems, #fluoroquinolones, #chloramphenicol, #tetracyclines and most aminoglycosides has been reported. On the other hand, the emergence of multi-drug resistant strain (MDR) has been a global concern [5,6]. Among various antibiotic, #aminoglycosides are considered as one of the critical agents. Aminoglycosides process is divided in two different steps, first absorption in bacteria which is an important process for bioavailability and second stage is the binding to the ribosome and then inhibition of protein synthesis [7].

Abstract

As the largest human #parasitic disease globally, #malaria is a significant public health problem [1-3]. In the world, 40% of the population are at risk of malaria infection. Programs have reduce the incidence of the disease and the associated mortality by 30% and 47% respectively between 2000 and 2013 4. However, although progress has been good, a lot remains to be done; 198 million cases of malaria and 584 000 associated deaths still occur every year [4]. Early and proper treatment of cases is one of the pillars of #malaria control. It has unfortunately come up against the emergence of resistance to the anti-malaria molecules used in #mono-therapies so far. The WHO had therefore recommended #bi-therapies with #artimisinin combination therapies (ACTs) [5,6] and asked countries to ensure a permanent monitoring of their effectiveness [7]. In Africa, most cases and #fatalities occur in non-immunized persons, #pregnant women and children. Every minute, a child dies of malaria. That represents 22% of all child deaths. But, with an increase in funds for malaria control, RDTs and ACTs has grown from 68% in 2005 to 75% in 2011, especially in sub-Saharan Africa. In Senegal, malaria related morbidity has dropped from 33.57% in 2006 to 3.07% in 2009. The National Malaria Control Program (PNLP) adopted ACTs in 2006 as the recommended treatment for uncomplicated malaria with Plasmodium falciparum [8].

Abstract

There is a persistent call [1-3] for multiculturalism and diversity of #theoretical and ethical frameworks in #clinical ethics support systems (CESS) around the world. Undoubtedly, there is a lot a strategic think-tank or support system situated within the clinical context, to reflect on the various ethical issues health #professionals face daily, can benefit from multiple #ethical reasoning. This will optimize health care delivery and improve patient-care. In order to achieve multiculturalism or cross-cultural and theoretical diversities in CESS, there is a need for a respectful engagement between medicine and social sciences and the #humanities. This engagement is mutually beneficial: the social sciences and the humanities can contribute a lot to medicine; similarly the humanities and social sciences can equally benefit from #medicine. Medicine can share with the social sciences and humanities some of the ethical challenges it contends with daily, and the social sciences and humanities can contribute - to medicine - rigorous scientific methodologies and moral reasoning derived from different cultures and traditions.

Abstract

When I was spending my childhood in southern Iran, I encountered people whose teeth were #brownish-yellow with white spots. I always wondered why their teeth are looking ugly. Are they not doing oral #hygiene? But when I got into the dental school, I realized that this was due to excessive fluoride in drinking water Interestingly, the percentage of dental caries in these individuals was much lower than others. From four years ago, following the popularity of #Varnish Fluoride in the international #dental community, the Ministry of Health in Iran has implemented the plan for children's fluoride therapy in primary schools in the country In the past few years, the project has been welcomed by parents and the dental community. Students in #elementary schools were examined every six months by dentists and treated by varnish fluoride for free. Every year based on this project, from the start of the school year, trained health #personnel will be active in the villages and in the cities so that school health educators and oral hygiene practitioners will cover all the elementary schools across the country for varnish fluoride therapy.

Environmental pollution exists since the very beginning of life, for which waste generation contributes. Waste management and #treatment is an important way to fight environmental pollution and contribute to human health improvments, provided that air, water, soil, waste and noise pollution all contribute in a negative way to increase environmental pollution provided the high level of urbanization and life standards nowadays. #Environmental pollution exists since the very beginning of life. However, it turned itself a serious problem today that threatens mankind survival [1]. It influences the air we breathe, the food we eat and the water we drink, causing the lack of natural resources, #extinction of plant and animal species, damages in global ecosystems and the earth is threathned by biochemical processes. In this sense, waste generation and management have a great impact on the environment and consequently over human health. Negative consequences of human activities include air and water pollution, uncontrolled #deforestation and their conversion into agricultural land, destruction of the ozone layer, global warming of the planet, climate changes, natural disasters, accumulation of various wastes (including radioactive), eradication of certain plant and animal species.

Abstract

An accurate clinical appraisal of #lymphoedema patients is often a challenge because the lymphoedema disability scales commonly used in rehabilitation are not completely suitable to a proper classification of such subjects. Therefore, the choice of how charging the single #lymphomatous patient to health public and private insurance systems worldwide is difficult. In order to address this issue, the authors defined and propose an evaluation system, called Lymphatic Clinical Severity Score (LCSS) that, analogously to the #Venous Clinical Severity Score used in the assessment of venous #chronic diseases, is aimed to assign the patient to a determinate clinical level. The LCSS score therefore results a useful parameter in evaluating whether to take in charge a patient is appropriate and how to do it. The LCSS has been successfully tested on 185 subjects with primary and secondary lymphoedema. The LCSS score determination can be repeated at the end of treatments and can be recalculated in the follow-up allowing a numerical temporal evaluation of the sickness.

Abstract

In the ancient Greek culture and #philosophy statements about the world had cosmological and #cosmogonic character. On the other hand, the second had always metaphysical (that is, mythical) nature of religious (like in #Orphism), quasi-religious (like in #Pythagoreanism) or secular (like in views of Plato - in spite of the fact that he referred to religious myths - as well as in Aristotle’s and #Epicurus' works) character.Thus, the pivotal aim of the first science (that is, the first philosophy); of philosophical theory or, in other words, the philosophy of the universe (cosmos); of theological astronomy or of astrophysical philosophy - which simultaneously constitutes a specification of the #philosophy of nature and an exposition of a significant part of Aristotle’s philosophy of myth - is the proper formulation and groundwork of the ideal (divine) foundation of metaphysics of the universe.

Abstract

Chronic psychological stress is a main #etiological factor for autoimmune disease by releasing neuropeptides activates inflammatory mediators such as IL-iβ, TNF-α, IL-6, and COX-2. These inflammatory mediators induce NF-KB transcription factor involved in chronic inflammation, immune modulation, cellular changes, and tissue damage, augments an autoimmunity. Beta-endorphins is an abundant #endorphin, natural #neuropeptides synthesized and stored in the anterior pituitary gland through hypothalamus mediated by autonomic nervous system (ANS), in response to stressful conditions such as physical exercise, yoga, and meditation. Beta- endorphins involved in treatment of autoimmune diseases by reduction of stress and inflammation. This article highlights about the role of #beta-endorphins on auto-immune diseases.Endorphins are endogenous #morphine, #neuropeptides produced from pituitary gland in response to stress and pain. There are three types of endorphins p- endorphins, #enkephalins, and dynorphins binds to mu, kappa, and delta receptors located on nerves and immune cells.
